Job Description
Overview
Coordinates patient care from diagnosis, through treatment, and after discharge. Assists in the coordination and planning of multiple healthcare services. Maintains communication with patients, survivors, families and health care providers to optimize access and monitor patient satisfaction. Facilitates a seamless transition of care in collaboration with patients, physicians, vendors and all stakeholders involved in the care of the patient. Serves as a liaison between referring providers, department providers and facility providers.
High school diploma or equivalent required. Bachelor's degree preferred.
0-3 years of experience required.
Knowledge of medical and social services required.
Clinical certification/licensure per unit/department specialty preferred.
Experience working as part of a multidisciplinary clinical team preferred.
