Responsibilities
The Diabetes Clinical Specialist will serve in a dual capacity, providing problem solving and clinical management of patients with diabetes and prediabetes at an expert level of practice in both the outpatient and inpatient setting. Provides guidance to the patient care team for the interdisciplinary phase of inpatient care. Is able to create and apply programs, conduct evaluations and accepted clinical research findings to secure clinical and program outcomes. The Diabetes Clinical Specialist’s primary focus is on the provision of direct and indirect patient care in managing the healthcare needs of the identified patient population with the possibility of cross campus coverage.

Qualifications
Minimum Education:
Bachelor of Science Degree required.
Minimum Experience:
Advanced training in diabetes preferred. A minimum of two year’s work experience in the education of person’s with type 1, type 2, gestational and prediabetes required. Continuous glucose monitor and insulin pump training preferred. Inpatient experience a plus.
Required Certification/Registration:
NYS licensed Registered Nurse or Registered Dietitian (RD or RDN) required. Must be a Certification Diabetes Care and Education Specialist (CDCES) or obtain the certification within two years of employment. CDC Certified Lifestyle Coach or obtain within one year of hire. BLS Required, ACLS preferred.
Physical Requirements:
Position will primarily be light work requiring lifting up to 25 pounds with occasional lifting and/or carrying objects up to 25 pounds. Occasional bending, twisting, stooping, kneeling, and reaching is required. Requires 80% sitting and 20% standing or walking.
Working Conditions:
Cross campus coverage based upon patient volume and necessity. Contact with patients under a wide variety of circumstances both in inpatient and outpatient setting. Subject to varying and unpredictable situations. Handles emergency and crisis situations. Flexible hours based on consumer needs. Ability to communicate and work cohesively with diverse groups.
