Work Hours/Shift:

Full Time, Days

You Will Be Responsible For:

* Work with Case Management in the acute care or community setting to help identify patients with post-acute care needs for Home Health Care.
* Participate in assessing patients for high risk of early readmission to the hospital
* Responding to requests for care from referring sources within the community in a timely matter
* Identifying clinical needs of patients so that a smooth transition of care can be planned facilitating good coordinated clinical care across the continuum
* Provide the patient/family with an informed choice of post-acute home health providers and securing their selection from the choice letter. (Presents a list of all available post-acute home care services to patient/legal representative per federal, state, and facility guidelines and regulations).
* Obtaining information to verify insurance coverage and inform the patients
* Identifying primary and/or secondary payer of services
* Participating in establishing the plan of treatment through direct communication with the patients physician by obtaining specific orders of care. May require an in person visit to the physicians office
* Explain the function of the home health team members who will implement the plan of treatment to the patient
* Coordinating the start of care activities with the Home Health Intake nurse or other agency designees
* Provides resource to interpret Medicare home health eligibility and guidelines
* Communicate pertinent information and instructions for specific physicians order to health team members, including completion of the interdisciplinary referral
* Facilitate the recording of the face to face encounter
* Provide information to physicians and the community on the benefits and alternatives home health services can provide
* Participate in and attend appropriate staff and committee meetings
* Document patient care coordinating activities according to procedures
* Makes appropriate referrals for patients not accepted for care
* Enhances personal and professional growth through attendance at workshops, conferences, health fairs, and participating in professional organizations
* Adheres to all department policies, procedures, standards of patient care, and departmental guidelines
* Participates in the orientation of personnel
* Complete special related tasks as assigned by the Administrator
* Will be crossed trained in the community and at the hospital

Qualifications
What You Will Need:

* Computer Training
* Associates Degree in Nursing
* Kansas RN License
* BLS through the American Heart Association
* 1-2 Years of Position Related Experience

Job Summary:

This position is accountable for identifying and evaluating patients in the hospital and/or community setting, by evaluating at risk patients for post-acute care home health services, performing patient and family data collection, conducting an assessment for appropriateness of and acceptance to home health services. This includes assisting the patient in the transition from the hospital or other facility to home or may include observing and evaluating the patient in the home setting. This position is also responsible for implementing specific patient planning activities such as collecting patient clinical and demographic data, assist planning of home health care with the attending physician and/or primary care provider, members of the multidisciplinary team and arranging support services. This position also provides community awareness programs to educate residents and referral sources of services offered by home health. This position needs a highly organized person who can multitask, possesses skills in home health eligibility &/or early hospital readmissions and who possesses strong communication skills.
