Work Hours/Shift:
Full Time Days
Monday - Friday
8:00AM 5:00PM
You Will Be Responsible For:
* Timely, accurate, and thorough completion of medical documentation
* Ability to give immunizations
* Appropriate documentation of patient information on intake (including identified standards for documentation i.e. mammogram, colonoscopy, smoking, etc)
* Ordering of pertinent medical supplies (including pharmaceuticals) and keeping inventory stocked
* Ability to utilize computer/computer software for pertinent patient documentation, providing documentation to patient as needed (i.e. patient plan). This includes pertinent training as necessary from time to time
* Adhere to policies/procedures for patient medications and testing refilling patient medications according to protocols (SMPG policies)
* Electronic communications with patients, when desired (in keeping with established policy standards)
* Review of patient information via huddles to comply with established PCMH measures, with appropriate follow-up and/or communication as necessary.
* Excellent communication (both written and verbal)
* Effectively coordinate patient care with pertinent referrals and/or follow-ups (within protocol/policy standards) and in keeping with PCMH established standards
* Meaningful use compliance to standards
* Timely management of patient needs/requests (including phone calls) in accordance both with policy and established PCMH standards
* Maintain pertinent logs of work performed as detailed by Clinic Manager/Director
* Adhere to clinic specific competencies (checklist)
* May be required to lead the clinical team in PCMH measures, for the clinic, as directed by Manager/Director
Qualifications
What You Will Need:
* Kansas LPN License
* Associates Degree or Graduate of a Technical School
* No Previous Experience Required
* BCLS obtained through the American Heart Association
Job Summary:
The Licensed Practical Nurse (LPN), under the direction of the Practice Manager/Director, provides nursing care to patients and assists with patient education. Assists provider during the treatment and examination of patient, and administers procedures and treatments as directed by the provider. The LPN should be prepared to handle the triage of patient calls; including prescription refills in accordance to refill protocols. The LPN is able to maneuver in the patient electronic health record (EMR) and documents patient vitals, and other pertinent information appropriately. Customer service skills are essential in all aspects of this position.
