Medical, spiritual, and emotional assistance helps patients and their families to ease the symptoms of life-threatening illnesses. Consider coming onto a team that is supportive, helpful, and meaningful. The ideal candidate will have one or more years post graduate experience in Outpatient Palliative Care. Candidates with a Palliative Care Certification are highly desired. 

Highlights about the practice:

Join a collegial team of two Nurse Practitioners, seeing 4-5 patients daily, with RN and MSW support.
100% inpatient and ability to grow the service line in our community.
On-site complex case management teams, social workers, chaplains, and partnerships with a Hospice House.
Full-time schedule; no call or weekend requirements.
Epic EMR.
