You Will Be Responsible For:

* Performs direct patient care at a highly professional level and on a timely basis to meet the daily needs of the inpatient and outpatient schedules as assigned by the Clinical Director.
* Performs comprehensive evaluations which include a chart review, use of accepted evaluation methods, and appropriate evaluation tools.
* Initiates evaluation within 48 hours of receiving the speech therapy order and completes the evaluation within two visits.
* Interprets test correctly and consults with patient, families, nursing, physician, and other appropriate resources in the development of the treatment plan as evidenced by chart documentation.
* Develops realistic short term and long term goals which have a high probability of being achieved. Demonstrate creativity, skill and efficiency in designing and implementing treatment programs for patients with all types of medical programs.
* Implements treatment program in accordance with the initial assessment and physicians orders to meet the patients needs. This shall be accomplished with no more than two incidences per year, without valid reason.
* Outpatient appointment assigned to the therapist will be met within 15 minutes if the patient arrives at the scheduled time. This will be accomplished with no more than two incidents per year, without valid reason, as determined by discussion with the therapist and department coordinator.
* Reviews medical records daily or more frequently as needed to gain information pertinent to services rendered by the Speech-Language Pathologist.
* Formally re-evaluates goals when goals require modification or upgrade, or when change in medical status necessitates it. Outpatients are reevaluated monthly to determine appropriateness of goals based on patients rate and extent of progress toward stated goals.
* Instructs the patient and/or family in appropriate home programs to address the patients needs and documents the rationale for the program as well as outcome of instruction.
* Recognizes a problem he/she is unable to hand alone, and elicits specific expertise of other hospital staff, physicians, and/or management.
* Exercises initiative and good judgment in identification of need for safe progression, modification or discontinuation of program when goals have been or are not being adequately approached as evidenced by documentation in the chart.
* Demonstrates the ability to assign priority to all or part of a patients treatment without compromising quality of treatment.
* Participates in discharge planning and makes appropriate recommendations regarding speech, language, cognitive, and/or swallowing needs post discharge.
* Appropriately delegates tasks to technicians.
* Plans and conducts modified barium swallow studies in conjunction with radiology department to assess swallowing disorders and facilitate treatment planning.
* Produces documentation appropriate for various levels and types of patient care within Therapy Services and accepts responsibility for the Therapy Services Department through generation of patient revenue.
* Documentation of treatment will be written by the next working day with 100% compliance.
* Each progress note and/or treatment plan will contain sufficient information in order for another Speech Language Pathologist to easily discern the following:

-Exact treatment to be given

-Modalities/procedures used

-Patient response

-Any projected change in plan

This will be carried out with 100% compliance on chart audit.

* For outpatient cases, a written progress report or discharge summary will be prepared and sent to the referring physician.
* Completes appropriate Medicare re certification form for Medicare outpatient reimbursement and discharge such patients when functional benefit of skilled intervention has been reached.
* Uses clear and concise documentation and adheres to hospital-approved abbreviations.
* Writes discharge report; gives home recommendations and home therapy program.
* Accurately charges inpatients and outpatients for treatments rendered and equipment/supplies issued according to department guidelines.
* Accurately completes charge sheet according to departmental procedure.
* Maintains productivity standards by adapting to changes in assigned daily caseloads.
* All evaluations, reevaluations and discharge documentation is completed within 48 hours.
* All evaluations and reevaluations are completed within 24 hours of the visit and filed in the patients medical record.
* Completes incident reports on any patients as appropriate and submits the report to the clinical director.
* Communicates appropriately with the Therapy Services Clinical Director, Clinical Coordinator, staff, medical staff, nursing staff and others in a professional and timely manner.
* Demonstrates cooperation and teamwork within the facility and communicates in a clear, concise, and constructive manner.
* Participates in departmental meetings, department performance improvement programs, training programs and in-services, and accepts departmental projects, supervisory responsibility, and assignments as requested by the Clinical Director and/or Coordinator.
* Contributes to the efficient and safe operation of the Therapy Services department. Conducts routine/general business necessary for quality and efficiency with department operation/ program development.
* Respects time limitations of others by being prepared and on time for discussions, meetings, etc.
* Organizes time effectively as demonstrated by the ability to prepare for expected and unexpected changes in schedule and utilizes department personnel appropriately.
* Keeps treatment plan within number of pre certified/ pre authorized visits without going over.
* Maintains physicians prescriptions up-to-date.
* Supervises and instructs clinical education students according to departmental guidelines.
* Participates in the development, maintenance of departmental policies, procedures and performance improvement activities.

Qualifications
What You Will Need:

* Must hold a Masters degree, M.A. or M.S. in SpeechLanguage Pathology, or its equivalent from an accredited college or university.
* Must hold a Certificate of Clinical Competence from the America Speech Language Hearing Association (ASHA), or be in process of initiating and/or completing the Clinical Fellowship Year (CFY).
* Must hold current Texas License or temporary license from the Texas State Board of Examiners for Speech Language Pathology and Audiology.

Job Summary:

Responsible for screening, evaluating, diagnosing and treating patients with Speech, language, cognitive and/or swallowing disorders along with appropriate counseling, communication education of patients, family and staff working with the patient.
