Compensation: $19.24 Hourly

POSITION PURPOSE

Responsible for engaging youth and their families into services, assisting them in accessing needed services and resources to achieve and maintain health and recovery.

ESSENTIAL FUNCTIONS - JOB SPECIFIC

Provide individual assistance to consumers in accessing needed mental health and medical services
Provide individuals assistance to consumers in accessing a variety of public services including financial and housing, including assistance on an emergency basis and directly helping to meet needs for food, shelter and clothing
Assist consumers to access and utilize a variety of community agencies to acquire ongoing social, educational, vocational and health and housing resources
Training, coaching and supporting in daily living skills, including housekeeping, cooking, personal grooming, accessing transportation, keeping a budget, paying bills and maintaining an independent residence
Follow up with consumers regarding appointments, completion of forms, returning forms or receipts and other similar activities
Assist client with transportation as clinically indicated; Driving is an essential function of this position
Act as a member of the Youth Emergency Room Enhancement (YERE) team
Collaborate with YERE team members, Adult ERE team members and community partners to facilitate treatment
Other duties as assigned

Job Requirements

EDUCATION

Master's degree in psychology, social work, rehabilitation/vocational/pastoral counseling, family therapy or closely-related field required

WORK EXPERIENCE

The completion of a practicum or at least one (1) year of experience under the supervision of a QMHP required
All experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities
A valid driver's license and agency established minimum automobile coverage required

LICENSURE/CERTIFICATION

Qualified Mental Health Professional (QMHP) required
PLPC, LPC, LMSW or LCSW preferred
