POSITION SUMMARY:
Collaborates with Social Worker/Case Manger in assessing, planning, evaluating and implementing the needs of the patient for discharge planning. This includes those who may have Medicare, Medicaid, HMO or private insurance to cover their stay at various units. Discharge planning is coordinated with physicians, nursing, patient and significant others who have an ongoing caring relationship with the patient. Is knowledgeable of criteria for Medicare, Medicaid coverage and that of HMO or private insurers. Responds to referrals same working day, referrals will be sent to HHC agencies, DME companies & SNFs. Provides IMs & patient choice, documents discharge planning progress notes to provide clear/concise record of discharge panning service assessments and interventions. Accepts delegations from Case Managers for succinct care coordination.

Qualifications:
POSITION QUALIFICATIONS (including required licenses/certifications, education and job knowledge/experience):
* 1-2 years of discharge planning, acute care, long term care, transitional care, or secretarial experience preferred; or other field of work with similar job-related tasks.
* Non-licensed experienced candidate or LPN licensure accepted
* Excellent computer skills with technological adaptability
* Knowledge of medical terminology and health care navigation
* Working knowledge of community resources available
* Ability to read and communicate effectively in English
* Additional languages preferred.
