Position Description
The Health Risk Assessment (HRA) Nurse - Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) will collect Health Risk Assessment data both telephonically and face to face. The LVN/LPN will initiate the formulation of a plan of care when applicable to be reviewed and verified by the Health Risk Assessment RN.
Essential Duties and Responsibilities
* Contacts patient and collects Health Risk Assessment (HRA) telephonically as required by plan
* Schedules home visits to collect HRA in accordance with plan requirement (which may include ride by visits)
* Performs home visit to collect Health Risk Assessment data. Maintains open lines of communication to HRA Nurse Manager regarding issues with data collection
* Communicates with Nurse Manager concerning additional needs patient may require to expedite process of getting the patient needs fulfilled
* Enters data collected in Trucare software according to applicable procedures
* Maintains accurate records providing detail of calls attempted and telephonic HRA
* Flexes hours of work with possibility of weekends to meet the needs of the client population and maintain a minimum productivity of at least 6-8 telephonic HRA or 60-80 attempted calls or combination of all.
REQUIRED Knowledge, Skills and Experience
* A current unencumbered State professional nurse license
* Current drivers license, good driving record and proof of insurance, and an automobile to be used for work
* The ability to make sound professional clinical judgment
* The ability to assess and document patient needs
* Proficiency in basic clinical skills
* Excellent verbal and written communication skills
* Proficiency in personal computer use including email, clinical software systems, word processing, and spreadsheet software
* Must have High Speed Internet access in home setting
* Must be able to work independently
Preferred Knowledge, Skills and Experience
* One year of experience as a professional nurse
