On Call Clinical Pharmacist (Phoenix Central)

Job Description

Role Summary:

To assist in the development and execution of numerous initiatives in collaboration with other care team members and providers to support the high quality of care, cost effectiveness of drug therapy and excellent customer service for Cigna Medical Group (CMG) customers.

Responsibilities:

Perform as a Clinical Pharmacist with focus on medication management of diabetes patients, hospital discharge medication reviews and Medicare Part D Stars initiatives

Assist in the support and monitoring of disease management and drug utilization management programs at CMG

Identify classes of drug therapy with potential for improvement in quality of care, increased efficiency, and improved cost-effectiveness

Potential one-on-one contact with providers to discuss quality of care, cost effectiveness and utilization management

Serve as a clinical pharmacy resource for CMG pharmacists, clinicians, support staff and the Cigna AZ Medicare Plan

Minimum Qualifications:

* Bachelors Degree in Pharmacy required; PharmD preferred
* Licensed Pharmacist in Arizona active with no restrictions
* Minimum of two years of experience in clinical pharmacy or completion of a clinical pharmacy residency program with a minimum of one year of experience in a managed care setting

Experience/training in diabetes management and patient monitoring required

Excellent analytical and problem-solving skills

Ability to communicate (verbal and written) and maintain effective interpersonal and working relationships with professional and support staff, clinicians and all customers

Ability to effectively teach/coach others

Experience with Microsoft applications including Outlook, Word, Excel and PowerPoint

Preferred Qualifications:

Possess current board certification

Bilingual (English/Spanish) preferred

Previous experience/training in medication reconciliation and Medication Therapy Management services
