Job Description

CLINIC RESPONSIBILITIES

Serves as a liaison with patients in the ambulatory oncology setting to ensure optimal clinical management in collaboration with APPs and MDs. This includes but is not limited to: patient teaching, prescription questions, education and enrollment in the Paxman cold-cap, and coordination of specialty medications.

Coordinates and completes prior authorizations, FMLA and/or disability requests as needed.
Coordinates (with assistance from APP/MD) VNA, hospice, and/or home infusion.
Facilitates patient in the enrollment of specialty medication programs, to include financial application guidance and educational materials within the specific disease centers.
Coordinates and assists with the newly diagnosed patients for the weekly multidisciplinary clinics.
Assist triage RN in the coordination of medical urgencies/emergencies to APP or MD as needed.

COMMUNICATION AND DOCUMENTATION

Provides ongoing patient management via telephone
Attends and participates in disease team meetings
Documents relevant patient information and current plan in EPIC
Collaborates with disease team providers
Collaborates with Infusion Nurses, Short Stay Nurses, Radiation Nurses and Research Nurses as appropriate
Collaborates with APP/MD in clinical research trials

Maintains steady communication with other members of healthcare team
Demonstrates sensitivity and patience in responding appropriately to the verbal and nonverbal communication of patients/families/and coworkers
Shows flexibility and compassion when dealing with a variety of patients/families in stressful situations
Acts independently and utilizes critical thinking in the resolution of clinical and non-clinical problems
Demonstrates reliability and is punctual and flexible in daily work schedule with coordination of coverage when away from unit

Current MA Licensure as a Nurse

BSN - Graduate of an accredited nursing program
OCN Certification strongly preferred
ONS Chemotherapy/Immunotherapy course completion required
Triage and care coordination experience required
Knowledgeable and experienced with a minimum of 3-5 years in medical-oncology nursing
