Work Hours/Shift:

Part Time, Days

20 Hours per Week, Flexible Hours

You Will Be Responsible For:

* Punctual with returning pages and phone calls, with completing documentation, timeliness in arriving at meetings and for patient treatments.
* Maintains a good attendance record, notifies supervisor of absences in a timely manner.
* Participates in team and staff meetings as assigned.
* Notifies PCS of patient schedule by 9:00 a.m. each day.
* Completes daily visit notes in a timely manner.
* Submits time sheets/DAS daily, in accordance with payroll schedule.
* Establishes effective rapport with patients, their families, and co-workers.
* Respects authority by following the appropriate channels of communication in resolving problems and presenting information.
* Readily accessible for new patient assignments.
* Demonstrates flexibility in accepting patient assignments.
* Communicates changes in the patients condition with the Case Manager or the Supervisor in a timely manner.
* Provides care with respect for the patients privacy and property.
* Demonstrates supportive and positive attitude of Agencys policy/procedure and practice to the community.

* Follows frequency and duration of HHA services established on HHA care plan.
* Demonstrates the ability to organize workload in order to meet productivity standards.
* Complete documentation within 24 hours.
* Completes documentation accurately the first time.
* Responsibly cares for tools/equipment issued by agency

Qualifications
What will you need?

* 1 to 2 Years of Position-Related Experience

Position-related experience includes:

* Preferably, have had at least one years employment experience as a Nurses Aide in an institutional setting.
* A minimum of 90 hours of training, including sixteen hours of supervised practical training by a qualified RN.
* Successful completion of a Home Health Aide competency evaluation.
* Good listening skills and ability to access desires of the patient in an accepting manner.
* Ability to function independently, problem-solve, prioritize and make decisions related to Home Health Aide responsibilities.
* Demonstrates sympathetic attitude toward the care of the sick and ability to deal effectively in a hostile or emotional patient situation.
* Effective interpersonal skills in relating to referral sources, co-workers and patients.
* Ability to communicate courteously with physician, clinical staff and patients.
* Knowledge and competency to perform required documentation and patient care procedures as a Home Health Aide.
* Demonstrates ability to read, write and carry out directions and to deal effectively with the demands of the job.
* Ability to use various tools, equipment and work aides as necessary to perform job duties.

Job Summary:

This position is accountable for assisting the health care professional by providing personal health care services and assistance with activities of daily living to the patient as specifically identified in a written assignment and necessary for the accomplishment of patient care plan goals.
