Primary Responsibilities:
* Make outbound calls and receive inbound calls to assess members' current health status
* Partner with the practice's administrative and clinical staff while managing patient appts and data between visits
* Create and maintain a professional and supportive relationship with the patient, provider and office staff
* Locate medical screening results/documentation to ensure quality measures are followed in the closure of gaps.
* Activities include data collection, data entry, quality monitoring, upload of images, and chart collection activities
* Identify gaps or barriers in treatment plans
* Provide patient education to assist with self-management
* Make referrals to outside sources
* Support members with condition education, medication reviews and connections to resources such as Home Health Aides or Meals on Wheels
* This is high volume, customer service environment.
You'll need to be efficient, productive and thorough dealing with our members over the phone. Strong computer and software navigation skills are critical. You should also be strongly patient-focused and adaptable to changes.

Required Qualifications:
* Associate degree or higher in nursing
* Current, unrestricted RN license in the state of Washington and Oregon licensure within the first 90 days on employment
* Must live within the western Washington area
* 3+ years of experience in a hospital, acute care or direct care setting
* Proficient in Microsoft Suite; Team, Word and Excel
* If you need to enter a work site for any reason, you will be required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener. When in a UnitedHealth Group building, employees are required to wear a mask in common areas. In addition, employees must comply with any state and local masking orders.
Preferred Qualifications:
* BSN
* A background in managed care, case management experience
* Certified Case Manager (CCM)
* Experience or exposure to discharge planning
* Experience in utilization review, concurrent review or risk management
* Previous experience in a telephonic role
