This position works closely with physicians to manage health problems for the nephrology patient population. The Physician Assistant is a licensed independent practitioner who is responsible for managing these health problems while coordinating health care for nephrology patients in accordance with State and Federal rules and regulations and the general standards of care (assessment of health status, diagnosis, development of plan of care and treatment, implementation of treatment plan, and evaluation of patient status). As allowed by state scope of practice regulations for Physician Assistants, this position may include, but not be limited to: performing dialysis rounds on End Stage Renal Disease (ESRD) patients; supporting a Chronic Kidney Disease (CKD) clinic; and hospital rounds and on-call coverage. The typical schedule will generally be Monday through Friday, but will also include weekend call coverage and dialysis rounds as required for patient care.

ESSENTIAL DUTIES & RESPONSIBILITIES:

70% - Oversee care coordination for the nephrology patient population and ensure the clinical team is meeting all relevant practice metric goals, including but not limited to:

* Functions independently to perform age-appropriate history and physical for complex nephrology patient

* Orders and interprets diagnostic and therapeutic tests relative to patients needs and Chronic Kidney Disease (CKD) state

* Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities consistent with scope of practice regulations

* Conducts weekly dialysis rounds on patients receiving hemodialysis, assessing their physical and psychological state

* Monitors the effectiveness of interventions and makes recommendations for treatment changes in collaboration with the health care team

* Facilitates the patients transition within and between health care settings, e.g. admitting, transferring, and discharging patients from the dialysis units and/or hospital

* Collaborates with multidisciplinary team members by making appropriate referrals

* Facilitates staff, patient and family decision making by providing educational tools

* Focuses on ensuring practice meets CKD and other quality metrics

10% - Provide leadership to the healthcare team, including but not limited to Dialysis Facility Administrators, Patient Care Technicians, Medical Assistants & Front Desk teammates

* Acts as a liaison between the healthcare team, physician and patient

* Knows, understands, implements, follows, and communicates to teammates appropriate roles and responsibilities as it relates to the Physician Assistant role and fosters a positive work environment

10% - Ensures the physician practice is delivering the highest levels of Service Excellence to physicians, patients and internal and external stakeholders, at all times

5% - Manages the fiscal resources of the practice and identifies areas of opportunity for operational efficiencies as it relates to the Physician Assistant role:

Assists in making recommendations to the practice on appropriate budgetary items relevant to the PA role (i.e., patient educational items, tools to enhance patient care, etc.)

5% - Other duties as assigned.

MINIMUM QUALIFICATIONS:

* Masters Degree as a Physician Assistant

* Experience with Physician Practice EHR

* Intermediate to advanced computer skills in MS Office suite (or similar programs), especially Excel and Word

* Private physician practice experience is preferred

* Travel up to 80% depending on business needs

Minimum of three (3) years' healthcare reimbursement experience preferred ; experience with admissions, billing, and collections preferred
Demonstrated knowledge of insurance rules, regulations and Coordination of Benefits for federal, state, and managed care payors in multiple states preferred
Basic computer skills and proficiency in MS Word, Excel, and Outlook required
