Work Hours/Shift:

* Full Time Days, M-F, 8a-5p

You Will Be Responsible For:

* Demonstrates critical thinking skills to troubleshoot and aid as a resource for the patient journey through the care continuum.

* Communicates effectively with regional physicians, management, staff, patients and families to ensure coordinated and cohesive execution of quality patient care in a timely manner.

* Assumes responsibility for professional growth and demeanor: attends all necessary committees or meetings relevant to the role. Collaborates with other clinical and administrative staff to optimize patient care delivery that are efficient and cost effective.

* Completes patient related/ educational assignments efficiently: demonstrates good organizational skills in planning/ implementing daily activities, demonstrates responsibility in cost effective use of hospital /patient supplies, equipment and resources, and facilitates early discharge of patients in clinical area of specialization.

* Assumes the responsibility as the vascular team advocate. Communicates effectively with physicians, leadership, staff, patients, and families to ensure the coordinated and cohesive care that is maintained.

Qualifications
What You Will Need:

KNOWLEDGE AND SKILLS REQUIRED:

Skill in the use of specialized equipment used in respective area of practice:

With physician support, teaching and counseling patients, families, and staff as needed

Possesses strong organizational skills, with the ability to prioritize, manage and complete multiple tasks or projects within specified timeframes

Strong Communication skills

KNOWLEDGE AND SKILLS PREFERRED:

Experience with multi-collaborative vascular Assessment

Experience with endovascular patient management

Training and knowledge in navigational and procedural modalities of Vascular patients

EDUCATION AND EXPERIENCE REQUIRED:

3-5 years experience in vascular/CV care

EDUCATION AND EXPERIENCE PREFERRED:

One year of Cardiovascular Lab experience

Interventional Radiology Lab experience

Peripheral Lab experience

LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:

Current valid State of Florida or multi state license as a Registered Nurse (RN)

Job Summary:

The overall goal of the Vascular Navigator position is to facilitate an evidence-based, multidisciplinary, multi-specialty approach to vascular care throughout the care continuum. Serves as a resource person for staff; responsible for collaboration, facilitation of patient care; provides direct clinical service to patients and families including care management, and patient education. This position will be the Expert of vascular patients and assist in the navigation of the patients. The Vascular Navigator gathers records, abstracts records and evaluates program data and uses this data to aid the Vascular Team establishing program goals. Collaborates and assists the CVI Leadership in developing and implementing projects that enhances the quality of the vascular care throughout the community.
