Virtual Care Ambassador (Community Health Worker Sr.)
Summary
The Virtual Ambassadors primary responsibilities center-around establishing trusting, supportive, collaborative relationships with patients and their families. A Virtual Ambassador will engage a panel of patients and encourage them to take an active role in their health by providing them with the tools necessary to make healthy lifestyle choices and adopt lifelong healthy behaviors. A Virtual Ambassador also builds individual, family, and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, informal counseling, social support, and advocacy. Virtual Ambassadors work alongside medical providers and behavioral health specialists in a collaborative, empathetic team.

Essential Duties and Responsibilities
Work with patients via tele-health visits, including video visit appointments, telephonic check-in calls, and email follow-ups
Point of contact for patient outreaches, appointment scheduling, appointment reminders, and ongoing patient support and education calls
Maintain patient contact information data, organized electronic records to track the status of patient contacts, and next steps/action items
Assist with the coordination of care across the continuum, such as: scheduling appointments with specialists, coordinating referrals, and sharing or transferring information with the patients internal and external care team
Help patients understand their chronic conditions, why they matter, and how they can manage them
Learn to employ confidence-promoting techniques in patient communication via motivational interviewing, action-oriented goal setting, and fostering patient self-advocacy
Utilize available community, government, and/or patient resources needed to address patients limitations or support interventions in the management of chronic or behavioral health conditions
Learn core insurance benefits to facilitate care navigation
Coordinate and collaborate with other virtual team members, including dieticians, pharmacists, palliative care specialists, social workers, and other specializes for patients as needed

High school diploma or general education degree (GED)
Minimum three (3) years work experience in a healthcare setting and/or experience working closely with community and government organizations, or any combination of experience
