$5,000 sign-on bonus after the completion of an introductory period! The Transfer Center at Community Health Systems is seeking experienced Licensed Clinical Social Workers to work night and day schedules within our triage telehealth team. This LCSW will be responsible for consultative behavioral health assessments in a telehealth setting, including the initial response, psychiatric triage assessment, designation of appropriate level of care/disposition, and initiation of placement/referral services. Our LCSW team works collaboratively to improve the flow of Behavioral Health patients from ER and inpatient settings into Behavioral Health programs, leveraging intake processes and telemedicine modalities to expedite the access to definitive behavioral health care.

The position works collaboratively to improve the flow of behavioral health patients from ER and inpatient settings into appropriate programs, leveraging intake processes and telemedicine modalities to expedite the access to definitive behavioral health care.

Requirements you need

Licensed in the state of Tennessee without restrictions (LCSW, LPC/MHSP, LMSW )with the ability to obtain licensure in additional states as required. Nurse Practitioners must have a Masters Degree in Nursing or a Doctorate in Nursing from an accredited college or university.
