The Patient Navigator assumes responsibility and accountability for the management of resources through interdisciplinary collaboration to achieve optimal patient outcomes. The Navigator coordinates research data, community physicians, hospital resources into a seamless model of access and care benefiting patients, participating physicians, and family members. Performs other related duties.

The Patient Navigator will coordinate all aspects of the department and Care Services including, but not limited to: patient and staff education, insurance, equipment needs and coordination of patients surgery with hospital and physician office, patient support group meetings. This position reports directly to the CNO with a dotted line to physician lead. The Patient Navigator will guide patients through the entire OR process from pre-admission to hospital and disposition. The Patient Navigator is the patient and family go to for questions and concerns and is easily accessible from beginning to end of the entire process. Shares twenty-four (24) hour responsibility for accommodating all aspects of patient care to include assessment of all patients discharge planning needs and coordination with case management of same to ensure a safe and timely discharge. The Patient Navigator serves to facilitate and enhance the patient's hospital experience. Performs other related duties. Bachelors degree in a health-related field required. Masters degree a health-related field preferred. Current Florida RN license required.
