DaVita is looking for a Full-Time Social Worker to join our Integrated Care team in New Jersey.
As the Integrated Care Social Worker you will work with a core integrated care team of two Nurse Practitioners and a care coordinator in addition to working with the larger care team.
You will serve as the primary case manager for an assigned patient census with complex health needs, focusing on care coordination and support to prevent patient hospitalizations.
In this role you will:
* Collaborate, coordinate, monitor, evaluate, advocate and educate!
* Collaborate with the Integrated Care Nurse Practitioner and greater care team; escalating medical issues requiring clinical attention to appropriate clinical teammates.
* Coordinator, monitor, evaluate and advocate for support services to meet patient behavioral health needs.
* Educate patient and patients care partners (when appropriate) on condition, treatment, and benefits of selected intervention
* Build strong relationships with patients, and investigate the social factors that are influencing their health (e.g., safety of home environment, financial constraints)
* Communicate and collaborate with a multi-disciplinary team which is comprised of Physicians, Nurse Practitioners, Nurses, Care Coordinators, Dialysis Center Teammates and the greater care team to share patients needs and ensure to provide the relevant support needed.
* Document all patient and provider contacts in medical record consistent with IKC standards
* Manage confidential information and/or issues using discretion and judgment
* Available to work non-standard hours when required
* All duties and responsibilities will be performed in a manner consistent with federal, state, and local laws and relevant IKC agreements.
* IKC Social Worker will only perform duties and responsibilities that are within their scope of practice and consistent with training and experience.
Qualifications:
* Masters Degree, and certification in Social Work (LCSW)
* 2 years of social work experience
* Must be able to work successfully in person, remotely, and via telephone or video conferencing technologies
* Excellent verbal and written communication skills
* Basic computer skills in MS Word, Excel, PowerPoint, and Outlook
* Functional proficiency with DaVita specific applications within 60 days of employment required
* Preferred experience working with renal or chronic/terminal conditions
* At least 2 years of healthcare social work experience
* Demonstrated knowledge of government and private insurance programs
* Available to work non-standard hours when required and make home visits when needed.
