Exciting work from home position available as a Patient Resource Specialist - Social Worker in our Health Management team. The Patient Resource Specialist - Social Worker participates in resource identification to promote the well-being of program participants. They function in collaboration with all members of the healthcare team to identify community and government resources while meeting a member's need.
As a Patient Resource Specialist - Social Worker you will:
Assist patients with expertise and knowledge of resource identification relevant to the member.
Use information gathering skills to assist with identifying available resources the member is eligible to receive.
Evaluate and provide information regarding the utilization
of community resources available for members and their families.
Identify funding sources, utilizing national foundations as a member resource when available.
Utilize your computer skills to document resources appropriately in members electronic medical record and provide written and oral updates to team members.
Provide a link between the member providers, payers and the community.

Required Qualifications
2+ years of prior related work experience in Social
Work or a community related discipline demonstrating the ability to plan, organize and evaluate patient well-being effectively.
Schedule: Monday thru Friday 8am - 5pm, with 1 flex day out of the week. The hours for the flex day would be a choice of 1) split shift 8am - noon and return for 5pm - 9pm or 2) 12pm - 9pm shift.
Preferred Qualifications
-Healthcare certificates or licensure
-Advanced experience in a health care community agency
-Intermediate Microsoft Office Skills
-Advanced degree/ Master's preferred
