Admissions Nurse - AdventHealth Hospice Care

Work Hours/Shift:
* Full Time Day Shift
* 10am-630pm Monday - Friday
* 2 Weekend Days Per Month
You Will Be Responsible For:
* Travels to resident site or hospital room of patient who has been referred to Hospice of the Comforter. Greets patient, caregiver and/or family member(s) and communicates the program and services available with Hospice of the Comforter.
* Partners with the Account Representative staff member to meet the needs of the patients and hospital staff to gain new referrals.
* Assesses the patient and familys emotional condition as to readiness for hospice. Answers questions and responds to concerns to encourage admission onto the Hospice of the Comforter program if conditions are justified.
* Assesses the patients physical condition, medical needs, current medications, and needs for special equipment. Determines patients primary physician, personal information, and special needs. If patients condition requires crisis intervention, psychosocial, chaplain or other special services, notify Hospice of the Comforter of need to assign appropriate team members. Obtain orders and initiate documentation for crisis care if required.
* Appropriately evaluates hospice eligible patients in a timely manner. Obtains approval from Hospice of the Comforter physician and the patients primary care physician to admit client to program. Communicates with the patients primary care physician with notification regarding admission of patient and care plan initiation with review.
Qualifications
What You Will Need:
* Education: Associates Degree is required; a Bachelors of Science in Nursing is preferred. An equivalent combination of education and experience may be substituted at the discretion of the Administrator.
* Experience: Minimum of 3 years registered nurse experience in home health and/or acute care nursing experience; including 1 year experience in the hospital is required. Hospice experience is preferred.
* Certificates and Licenses Required: A clear/active Florida Registered Nurse (RN) license is required, Certified Hospice and Palliative Care Nurse certification is preferred.
Job Summary:
The Admissions Nurse is responsible for responding to and visiting with patients who have been referred for admission to Hospice of the Comforter. The Admissions Nurse informs the patients, caregivers, and family members of the hospice services available with the organization and assesses patient for admission to services. In addition, the Admissions Nurse is responsible for visiting current Hospice of the Comforter patients who have been admitted to the hospital and communicates patient status with the respective interdisciplinary team.
