Responsibilities
Collaborate directly with agency Director and Clinical Leadership team members (Medical Director (MD), Clinical Manager (CM)) to ensure high level client service, clinical excellence, quality, and adherence to policies and procedures in accordance with The Southern California Home Health and Hospice Way. Accountable for supporting clinical leaders in the visit offices in effective execution of: episodic case management, high quality clinical care, and professional development for the clinical team.
Qualifications
MINIMUM QUALIFICATIONS:
* Holds a current license in good standing as a Registered Nurse in CA.
* Four (4) year college degree preferred, or a combination of education and experience.
* Three (3) to five (5) years of Home Health or Hospice Clinical Leadership experience.
* Demonstrated record of strong interpersonal, communication, and leadership skills.
* Demonstrated record of goal achievement.
* Demonstrated record of successfully taking on more responsibility with positive results and ambition to grow past current position.
* Competence in basic PC skills required to perform job functions.
* Completed Continuing Education Requirements (CEUs) as required by regulation.
* Completed criminal background and federal/state program exclusions checks.
* Clean driving record.
* Pass drug screen
* Ability to read, write and effectively communicate in English.
* Demonstrated expertise in episodic case management, high quality clinical service delivery, and multidisciplinary team management.
* Demonstrated record of developing staff into Clinical Leadership roles (CM/RM) or other promotional opportunities.
* Working knowledge of PDGM and Hospice reimbursement.
* Strong understanding of CoPs, and Title 22.
* Demonstrated expertise in all areas of responsibility.
* Effective presentation skills and ability to influence an audience.
