Job Description
Havasu Regional Medical Center
Under the Direction of RN or MSW, the Discharge Planner will assist in providing appropriate and timely discharges from the hospital, by sending referrals to community resources, such as Home Health, Hospice and Skilled Nursing Centers. Discharge Planner will follow up and coordinate the acceptance and obtain choice letters associated with such referrals.
Reports to: Director of Case Management
FLSA: Exempt
Job Requirements
Minimum Education
High School Diploma required.
An associate's degree from an accredited program in Social Work or closely related field preferred
A bachelor's degree from an accredited program in Social Work or closely related field preferred
Minimum Work Experience
Minimum one year of experience in a social services field; or in a healthcare setting. Acute Care Hospital experience is preferred.
Required Skills
Excellent organization skills.
Excellent oral and written communication skills.
Empathetic towards the needs of others.
Ability to handle stressful situations with physicians, patient, and family.
Ability to report social or financial issues to the RN or MSW to resolve problems encountered in discharge planning process.
Ability to work as a part of a multidisciplinary team and to facilitate close working relations and cooperation with community resources.
