Community Engagement Navigator is responsible for engaging with Anthem members to remove barriers to Social Determinants of Health (SDoH) and acting as the communication liaison between Anthem and referral sources.

LOCATIONS:
This is a field-based position that will serve our members in the Commonwealth, with up to 80% of local travel within an assigned area. When you are not in the field, you will work remotely from home. Occasional office visits for meetings and/or training are required. Note: Temporarily not doing field work. Telephonic/virtual outreach only due to the pandemic.

Paducah

Louisville

Madisonville

Hopkinsville

Primary duties may include, but are not limited to:
Utilizes screening tools to identify appropriate programs and services for members.
Engages with members who have been identified as having SDoH barriers, such as housing, food insecurity, transportation, employment, education, and criminal record expungement
Works with members to establish goals and next steps for addressing barriers.
Interfaces with the Population Health team to ensure the individualized plan is aligned with existing or needed case management efforts.
Supports the member in selecting and connecting them with the most appropriate provider, program or service to meet their needs.
Promotes and educates on health plan programs and services; including fulfillment of SDoH program Value Added Services;
Acts as a liaison between our Population Health team and local, Commonwealth, or federally funded programs.
Establishes and maintains positive relationships with community-based organizations and other key partners
Preferred experience and knowledge of HIPAA standards, local social service organizations and community resources to assist members with SDoH needs.

Qualifications

Bachelors Degree in Public Health, Social Work, Rehabilitation Counseling or related field and 2-3 years experience with marketing, job development, job placement or social work or the equivalent in education and experience.
Community Health Worker certification required within one year of employment
Travel required.
Computer skills: You will receive training on Anthem specific applications, however, you must be comfortable working on a laptop with web-based applications. You must have experience working with MS Word, Outlook, and Excel. If you are not computer savvy this may hinder your ability to be successful in this position.
