Location: US:WI:MILWAUKEE at our FROEDTERT HOSPITAL facility.
FTE: 0.900000
Shift 1. M-F; Four day work week is an option as it meets the business need but 5 days one week 4 the next may be required for patient care.
General office hours. No weekend/holidays required.
Summary:
The Kidney Transplant Coordinator will act as a liaison between families hospital staff and patients who are pre and/or post Kidney transplant. Responsibilities include participating in the clinical management of the patient with all necessary disciplines to coordinate patient services and education across the continuum of care.
Responsibilities:
Minimum of 3 years of acute care or dialysis experience is required. Experience relevant to Nephrology/Transplant subspecialties is preferred. Associate's degree in Nursing with a minimum of 10 years of relevant experience is required or Bachelor's degree in Nursing with a minimum of 3 years of relevant experience is required. Bachelor's degree in Nursing is preferred. Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact). Certified Clinical Transplant Coordinator (CCTC) certification or Certified Clinical Transplant Nurse (CCTN) is required within 3 years of date of hire. Incumbents in the role as of 3/1/2020 must obtain a certification within 2 years.
Other information:
Hands on patient experience in an intensive care setting. High degree of organizational skills and excellent communication skills; ability to work independently and take programmatic initiative. Knowledge of data systems outcomes data and other clinical performance data.
