Memorial Medical Center
Manages patient's transition through the healthcare system and transfers accountability to the appropriate person or agency upon discharge. Performs admission, concurrent and retrospective review for all patients using Interqual Criteria. Provides necessary information to 3rd party payers, physicians, staff, patients and families. Accountable for accurate resource utilization and facilitation of improvement in quality care for assigned caseload. Provides services to patients throughout the lifespan including all age groups.
Job Requirements
MinimumEducation: Graduate of a Social Work BSW or MSW program.
MinimumExperience: Must possess familiarity with case management issues,discharge planning and utilization management.
Licensure/Certification: Mustpossess a current New Mexico license as a Social Worker. InterqualCertification or Case Management Certification preferred.
