As a Community Transitions Staff Nurse, If you reside in one of the following time zonesPST, MST, HST, AST, you will have the flexibility to work from home* as you take on some tough challenges.

Primary Responsibilities
* Completes post discharge assessments evaluating members for clinical and social needs timely and effectively
* Follows model of care requirements as outlined in policies, procedures and job aids
* Educates members on disease process and medications to include completion of the Medication Reconciliation – Post Discharge
* Evaluates members for ongoing needs and completes referrals to longitudinal programs as appropriate
* Manages, communicates and updates plan of care with members
* Collaborates with other programs and departments as necessary
* Collaborates with Primary Care Physicians regarding medication concerns, questions or other member needs

Required Qualifications
* Current unrestricted RN License in a compact state
* RN that resides and is licensed inPST, MST, HST, ASTTime Zone
* 2 years case management, managed care, home health experience OR acute care clinical experience
* Basic level of experience with Microsoft Word, with the ability to navigate a Windows environment
* Internet access
* Must have a designated workspace and access to install secure high speed internet via cable in home
* If you need to enter a work site for any reason, you will be required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener. Employees must comply with any state and local masking orders. In addition, when in a UnitedHealth Group building, employees are expected to wear a mask in areas where physical distancing cannot be attained.
Preferred Qualifications
* Bachelors Degree
* Certified Case Manager (CCM)
* Experience working in a call center environment
* Experience in discharge planning, utilization review, concurrent review and / or risk management
* Strong organizational skills and multitasking abilities
Work Schedules
* Nonexempt schedules
* Monday-Friday 8:30-5:00pm in your time (EST/CST) with opportunities for overtime
* May require weekend work

Colorado Residents OnlyThe salary/hourly range for Colorado residents is $53,300 to $95,100.
