Positions in this function require education/certification/licensure other than an RN, such as BSW, MSW, LCSW, etc. Positions are responsible for providing clinical and medical management services, including case management, health assessments, interventions, and discharge planning. May identify, coordinate or provide appropriate levels of care under the direct supervision of an RN or MD. This function does not include Health Coach, Health Educator and Health Advocate roles that do not require an RN. Those roles are found in the Wellness Coach Management function.


Primary Responsibilities

Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of care
Identify and initiate referrals for social service programs including financial, psychosocial, community and state supportive services
Manage the care plan throughout the continuum of care as a single point of contact
Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
Advocate for patients and families as needed to ensure the patients needs and choices are fully represented and supported by the health care team
Expect to spend about 80 of your time in the field visiting our members in their homes or in long-term care facilities. Youll need to be flexible, adaptable and, above all, patient in all types of situations
Generally, work is self-directed and not prescribed
Works with less structured, more complex issues
Serves as a resource to others

Required Qualifications
* Masters degree in Social Work or other related field
* 2 years experience in long-term care, home health, hospice, public health or assisted living
* 2 years of experience working with MS Word, Excel and Outlook
* 1 years of clinical or case management experience
* Access to reliable transportation
* If you need to enter a work site for any reason, you will be required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or similar UnitedHealth Group-approved symptom screener. When in a UnitedHealth Group building, employees are required to wear a mask in common areas. In addition, employees must comply with any state and local masking orders
Preferred Qualifications
* LCSW
* A background in managing populations with complex medical or behavioral needs
* Experience with electronic charting
* Experience with arranging community resources
* Prior field based work experience
