POSITION SUMMARY:

The eICU Clinical Pharmacist Coordinator works with an eICU team in a central eICU facility providing pharmaceutical care to ICU patients at various remote hospital sites. They work collaboratively with eICU providers to provide collaborative drug therapy management to optimize medication therapy, improve patient outcomes and improve cost efficiencies. This includes medication reconciliation, medication order verification, therapeutic interchange, renal dosing, and other appropriate clinical interventions as well as providing drug information. The Clinical Pharmacist Coordinator supervises other pharmacists providing eICU services. They work with other eICU staff and interface with on-site facility providers and staff. They work flexible shifts. They provide leadership to standardize pharmacy practices, promote medication safety and collaboration within Steward Health Care System. This role requires knowledge of a pharmacist scope of practice, current state(s) regulations, CMS conditions of participation, accreditation standards, and other Federal or State regulatory agency requirements specific to medication order processing.

KEY RESPONSIBILITIES:

* Oversees the day-to day operations of eICU pharmacy including compliance with company polies and procedures and appropriate record keeping.
* Assumes a leadership role in the maintenance of the patient-focused eICU.
* Creates, develops and implements policy and procedures regarding critical care drug therapy prescribing and monitoring.
* Provides comprehensive medication assessments and collaborates with the integrated healthcare team to provide quality patient care primarily through the use of telephonic, computer and video interfaces
* Assigns, prioritizes and evaluates the work for multiple facilities. Designs programs for use across various e-ICUs and electronic health records
* Delivers pharmaceutical care to patients using the medication use process (selecting, recommendations on prescribing and administration, dispensing and monitoring of medications) through an integrated system approach with prioritized activities including documentation of activities
* Provides appropriate therapeutic drug monitoring, drug regimen review and consultation when requested.
* Evaluates and addresses drug-drug interactions, drug-food interactions, allergies, contraindicated medications, inappropriate therapeutic regimens, adverse drug reactions and medication therapy management (pharmacokinetics, sedation and pain management)
* Monitors glycemic response to therapy as well as nutrition, electrolyte and fluid replacement and response.
* Monitors patients with stable and unstable organ function (such as renal and hepatic) that may affect drug dosing and response and provides recommendations for drug dosing modification and monitoring
* Supports remote medication management activities including order processing, verification and clarification in a manner consistent with peers in like shifts.
* Processes clinical interventions consistent with peers in like shifts, investigates and completes Medication Event Reports.
* Participates in developing the skills of other pharmacists and affiliated healthcare team
* Collaborates with clinical leadership to participate in drug use initiatives and assure appropriate use of pharmaceuticals.
* Conducts educational activities, applies principles of performance improvement and participates in committee activities as appropriate.
* Communicates effectively between interdisciplinary healthcare members
* Adheres to local, state and federal laws regarding the profession of pharmacy practice
* Identifies how drug costs may be optimally managed through appropriate use of drugs in the ICU and through implementation of cost-containment measures.
* Evaluates antimicrobial use including results of cultures and susceptibilities following the precepts of antimicrobial stewardship.
* Participates on inter- and intra-disciplinary committees in order to integrate and coordinate the activities of Pharmacy, Nursing, Unit coordination and the physician staffs.
* Supervises and directs work of interns, residents and students, where applicable.

REQUIRED KNOWLEDGE & SKILLS:

* Knowledgeable and experienced in operating in high performing contemporary clinical pharmacy services including critical care services including knowledge of use of vasopressors, sedation and pain management as well as nutrition, electrolyte and fluid management and antimicrobials.
* Demonstrates an ability to utilize effective communication, presentation, and analytical skills, exhibiting a record of self-initiative, strategic thinking, successful planning, and innovation.
* Colleagues consider an expert on critical care drug therapy protocols and literature.
* Has demonstrated ability to effectively implement change and reduce medication costs within a multihospital environment.
* Able to work collaboratively with physicians, advanced level providers (PAs, NPs), nurses and pharmacists.
* Knowledgeable and experienced in various areas of pharmacy, including but not limited to: clinical functions and operations, systems, and technology
* Demonstrates ability to lead and manage professionals and others collaboratively across multiple sites.
* Proficient with using computers as needed to monitor and support clinical performance
* Strong understanding of statistical analysis, clinical study methodologies and outcome based measurement.
* Excellent written and verbal communication.
* Ability to establish relationships through face to face, telephonic and computer assisted meetings.

EDUCATION/EXPERIENCE/LICENSURE/TECHNICAL/OTHER:

1. Education: Graduate from an accredited College of Pharmacy; Advanced Professional degree (e.g. Master of Science, or PharmD, or PhD)
2. Experience: PGY1 or 3 years of like experience required. PGY2 in Critical Care or Emergency Medicine & Management experience preferred.
3. Certification/Licensure: Pharmacist License, in good standing with a Board of Pharmacy
4. Software/Hardware: Microsoft Word, Excel, PowerPoint, preferred
