Compensation: $56,929.60
POSITION PURPOSE:
The Healthcare Home Regional Nursing Director directs and manages a regional team of nurse care managers, providing guidance and clinical supervision on all aspects of healthcare home policy/procedure and processes.
ESSENTIAL FUNCTIONS - JOB SPECIFIC:
1. Champion HCH practice transformation - work collaboratively with CPRC/CSTAR/Outpatient Nursing and other clinical leadership to ensure that health home activities are integrated into other clinical programs as appropriate:
* Maintain communication with other department directors and supervisors regarding program/client needs
* Provide feedback to Healthcare Home Director regarding issues, developments, and needs of region
* Work with HCH Directors on analysis of data and actions to be taken to ensure Continuous Quality Improvement
* Assist other clinical teams in learning various software programs necessary for HCH data entry and management
* Provide or arrange training on health topics for all staff
* Review Health Screens for CPRC clients not enrolled in HCH
* Ensure compliance with Compass goal for completion of metabolic syndrome screening
* Provide consistent positive feedback for completion efforts
* Manage supply inventory and ordering and compliance with testing protocols for region
2. Lead and manage team of Nurse Care Managers:
* Provide administrative and clinical supervision
* Ensure adequate staffing patterns to meet needs
* Orient new personnel as needed
* Disseminate information regarding policies and procedures to staff
* Conduct AIM discussions with direct report staff
* Conduct regular supervision and monthly team meetings with NCMs, and support staff opportunities to facilitate professional growth
* Ensure that all components of Healthcare Home are implemented
* Ensure proper documentation of all HCH nursing services
* Provide backup to any direct report position as needed
3. Ensure optimal enrollment to slot capacity by overseeing admission/transfer/discharge of enrollees
4. Attend Compass Metabolic Compliance Meeting, HCH Regional Integration meetings, HCH Leadership meetings, agency and state level HCH meetings and trainings, and other meetings as assigned
5. Other duties as assigned

Job Requirements
EDUCATION
Associate degree required
BSN preferred
WORK EXPERIENCE
Two years of clinical supervisory experience preferred
LICENSURE/CERTIFICATION
RN license required
