Job Summary

Provides chemical dependency counseling and chemical dependency education for residential, partial and IOP patients and their families; provides case management services; communicating and working with all clinical staff, utilization review specialists, managed care organizations and referral sources.

Job Description

Minimum Qualifications

Education: Bachelors in a behavioral health related discipline; Masters preferred

Licenses/Certifications: Licensed Chemical Dependency Counselor (LCDC) in the State of Texas

Experience / Knowledge / Skills:

* Five (5) years of experience as a licensed chemical dependency counselor in the United States or
* Masters Degree in a behavioral health-related discipline and at least three (3) years of experience as a chemical dependency counselor and
* Two (2) years of experience of clinical program management in an intensive outpatient, residential or inpatient program setting

Principal Accountabilities

* Facilitates treatment from the screening process through discharge planning according to the age/needs of the patient population served.
* Facilitates individual sessions, process groups, patient and family education lectures, recording all pertinent patient information including individual and family sessions, Master Treatment Plan updates, and any other significant incidents daily.
* Participates in crisis management, motivational interviewing, life skills training, working directly with Employee Assistance Professionals, attending Return to Work meetings, and all corresponding paperwork.
* Considers the diverse needs of each patient through the entire continuum of care, providing and ensuring the highest level of service according to established service standards, optimizing the patient experience.
* Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
* Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.
* Other duties as assigned.
