Collaborate with the Clinic Network to identify and call patients who are eligible for wellness visits and/or have fallen behind on recommended care and screenings. Demonstrates compassion, collaboration, communication, and appropriate clinical judgment in patient care. Responsible for conducting outreach, scheduling, and verification activities for preventative services and Medicare Annual Wellness visits. Calls patients to review and record intake questions for the electronic record. Obtains patient summaries from payers. Provides direct support to providers in delivering superior patient care by documenting all patient communication in electronic record. High school diploma or GED required. Must have excellent communication skills.
