The Case Manager facilitates efficient, cost-effective, quality care including appropriate utilization of resources in order to implement a plan of care to meet the needs of the individual and/or patient. The Case Manager assesses the patient and family for clinical appropriateness for the setting and establishes plans for effective allocation of resources and management of the continuum of care. The Case Manager assesses, plans, implements, and reevaluates an individuals care and discharge needs in order to promote improved patient outcomes.

What Does a Case Manager Do? As a Director of Laboratory with Lower Keys Medical Center, you will spend your day

* The Case Manager uses clinical knowledge, communication skills, and independent critical thinking towards interpreting medical data, criteria, policies & procedures to guide the right care, at the right time.
* Coordinates and communicates with insurance providers, the treatment team, and other parties to facilitate optimal care and treatment.
* Completes medical reviews, appeals, and discharge planning within established timeframes.
* Prefer an experienced Case Manager, but would train the right individual.
* Prefer certification in Case Management (CCM, ACM or RN-BC)
* Coordinating the overall interdisciplinary plan of care for a patient, from admission to discharge.
* Acting as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively.
* Relying on experience and judgment to plan and accomplish goals.
* Performing a variety of tasks.
* Reporting to Director of Case Management.

Which Qualifications Do You Need? In order to set you up for success, you need the basics of

* Current Florida Registered Professional Nurse in accordance with Chapter 464.061 of Chapter 464 of the Florida Statues.
* A minimum 2 years in the medical industry
* Familiar with a variety of the field's concepts, practices, and procedures.
