We are seeking a Nurse Care Facilitator for our Access Response Center to ensure that we continue to provide all patients with high quality, efficient care. As a Nurse Care Facilitator, you will contribute to the companys mission and vision by providing nursing support to the ARC by reviewing the medical and behavioral health needs of patients. Under supervision of the Division ARC Manager and Director, you will review the medical presentation of patients referred for inpatient services by internal and external customers for behavioral health unit capability, interact with providers to facilitate admission orders and interface with third-party payers.

What you will do in this role:

* You will be responsible for assessing and reassessing the needs of your patients through verbal discussions with referring staff and evaluating medical/behavioral records
* You will review referrals for inpatient behavioral health treatment from internal and external customers
* Coordinate interested parties and physicians through patient referral inquiry and admission processes
* You will interact with admitting behavioral health providers to share clinical information about referred patients and take admitting orders by telephone
* You will function as the liaison to behavioral health units by remaining abreast of facility/unit specific acuity
* You will serve as the ARC internal expert regarding the medical needs of behavioral health patients and you will be readily available for consultation with ARC clinicians/colleagues and facility staff when needed
* You are responsible for completion of behavioral health assessment with patients through the use of tele-mental health technology

What qualifications you will need:

* Associates Degree in Nursing
* Minimum of 3 years in a behavioral healthcare setting performing assessments preferred
