The SDOH project coordinator will be responsible for oversight of employment strategies. He/she will also support housing and educational related initiatives to support and improve the lives of our Medicaid members and community.
If you are located within the state of Kansas, you will have the flexibility to telecommute* as you take on some tough challenges.
Primary Responsibilities:
* Develops overall employment strategies
* Facilitates and cultivate relationships with senior staff at local Work Force Centers, Colleges, local continuum of care agencies, homeless shelters/providers, and Community Based Organizations
* Serves as subject matter expert on employment for all areas
* Supports Housing Navigator
* Provides strategic consultation and thought leadership to influence and make recommendations to State and Local agencies/authorities for new program initiatives and innovation process improvements
* Represents the health plan to lead strategic initiatives with external employment agencies/providers focused on increasing access to employment in the community
* Drives specific employment initiatives and support through collaboration with clinical teams to define, develop and monitor programs and impact on health outcomes, quality initiatives and cost impacts
* Seeks and develops trainings for Health Services staff to improve team's capacity to engage members and connect them to the appropriate resources
* Creates linkages to support members in efforts to find and maintain stable employment through research and networking within communities; occasionally assisting members with employment and unemployment applications
* May support members at the one to one level
* Responsible for day to day monitoring, testing and assessments of pilots to reduce costs, improve quality outcomes and comply with state requirements
* Consults with other markets to share local initiatives and successes relative to employment strategies

Required Qualifications:
* Bachelor's Degree
* 3+ years of experience in any of the following: employment programs, community development programs, working with underserve communities, supportive housing services
* 3+ years of experience developing or supporting employment and or educational programs for underserve communities
* Knowledge of federal, state, and local laws and policies that regulate employment and employment programs
* Good understanding of the importance of socioeconomic drivers of health, social justice and equity
* Intermediate proficiency and presentation skills with Microsoft Office applications
* Proven negotiation, problem solving, planning and decision making, and facilitation skills
* Ability to travel statewide 50% of the time via car (valid driver's license)
