At UP Health System Portage, providing an excellent patient experience is at the heart of everything we do. PortagePointe is actively seeking engaging, positive, eager Elder Associates (CNAs). PortagePointe is a beautiful Eden Alternative home for 60 Elders from our community. There are 5 different homes where CNAs have the opportunity to positively impact the lives of Elders through engaging in daily care, eating with elders, walking in the beautiful garden, and participating in activities in and outside our homes, providing encouragement for each Elder to reach their maximum functional ability. Because Elder Associates (CNA) at PortagePointe play a vital role in the lives of our Elders, CNA candidates we require exemplary customer service fitting of our high standards and environment of care. An Elder Associate feels good at the end of the day knowing the work theyve done is appreciated by the team and Elders because they put our patients at ease. Job Requirements
High school diploma or equivalent required
Successful completion of nurse aide training program required
Current state nurse aide certification required
CPR certification required
Annual Influenza Vaccination required

Our Operating Groups

Our Health Support Center
Our Health Support Center (HSC), in Brentwood, Tennessee, has subject matter expertise in every area of healthcare operations and provider services. The HSC is organized to bring resources to bear across the continuum of care, from before a patient accesses healthcare services to after they are sent home, while keeping quality and compliance at the center.
On a daily basis, our group leaders and subject matter experts are working with our local markets to help develop and implement the right strategies that ensure healthcare thrives in their communities.

Physicians are a critical access point to care in our communities. We work to build and maintain collaborative relationships with both our employed physicians and a broad network of affiliated providers, and we provide a full spectrum of practice management and physician support to help secure the delivery of quality care today and for the future.
