POSITION PURPOSE

The Hospital Access Liaison (HAL) assists individuals who are recently being discharged from the Emergency Department (ED) or inpatient unit that need immediate assistance with linkage to services and resources in the community. The focus will be on those individuals who are at high risk of hospitalization/re-hospitalization and/or risk of suicide and who do not meet criteria for another service at the time of assessment.

ESSENTIAL FUNCTIONS - JOB SPECIFIC

Provide quick response and care for individuals discharging from hospital, with preference of co-location on site to have quick access to individuals being referred to the HAL
Use short-term, community-based interventions for up to 90 days after initial contact
Expected to travel to meet individuals in the community, when needed
Communicate regularly with hospital staff to ensure continuity of care, discuss referral workflow and work out any issues
Make follow-up calls and/or outreaches to referred individuals, as appropriate
Attend monthly meetings and weekly clinical supervision with Director of Access & Urgent Care Services.
Maintain agreed upon level of productivity
Provide culturally competent services to meet the unique cultural needs of each referred individual
Be knowledgeable of all services provided within Compass Health
Be knowledgeable of other human/social services agencies for purposes of referral if Compass is unable to provide a service and maintain documentation of such referral
Maintain client records, following agency and state regulations
Ensure all documentation is written in proficient and professional manner
Work collaboratively with programs to ensure the individual is referred, when appropriate and eligible
Collaborate and communicate with caller's multi-disciplinary treatment team, if existing client is referred
Other duties as assigned by supervisor
Flexibility with shifts may be required

QUALIFICATION(S): EDUCATION

Master's degree in social work, psychology or related field required

QUALIFICATION(S): WORK EXPERIENCE/TRAINING/ADDITIONAL REQUIREMENTS

At least three (3) years experience working in behavioral health required
Working knowledge of DSM-V required
A valid MO driver's license with agency-established minimum automobile coverage required

QUALIFICATION(S): LICENSURE/CERTIFICATION

PLPC, LPC, LMSW, LCSW, PLP or LP preferred
