Position Description

The HRA Technician will be responsible for sending, tracking all HRA reminder letters, receiving/routing follow-up phone calls, assisting with scheduling HRA visits with all clinicians as indicated, and collecting Health Risk Assessment data where allowed by Health Plan.

Essential Duties and Responsibilities

* Performs health risk assessments in accordance with plan policies
* Maintains the HRA reminder tracking log and assessment visit schedule
* Performs outreach calls and warm transfers to HRA assessment nurse
* Performs filing, copying, faxing, and other administrative duties
* Enters all HRA completed on paper into Tru-Care or other software as required
* Develops, maintains, and sends reports in accordance with plan policy
* Maintains open lines of communication with all members of the healthcare team
* Communicates with staff and patients
* Follows agency policies and procedures
* Primarily works in an office environment and answer phones according to agency policy
* Flexibility to work swing shift hours including weekends and evenings as needed to support contracted health plans.
* Performs all other duties as assigned by management, HRA Nurse and/or Administrator
* Completes additional tasks outside of the above responsibilities as assigned.

REQUIRED Knowledge, Skills and Experience

* A High School diploma or equivalent
* Computer proficiency and ability to type 40 wpm accurately
* Must have good knowledge of excel and ability to work in various medical software systems
* Excellent communication and organizational skills

Preferred Knowledge, Skills and Experience

* Associates Degree
* Knowledge of medical terminology and preferred experience in a health related field.
