Job Summary
* PT position on night shift, Saturday and Sunday, 7p to 7a.
* 6 months to 1 year patient access experience required.
* Medical terminology required.
SUMMARY: This position will assist the Clinical staff in patient flow and appropriate patient placement according to diagnosis and service line. This person will collaborate with Patient Access Services and all hospital departments in appropriate patient and census management. When necessary, this person will also serve as a liaison to physicians, patients, families and staff as appropriate.
DUTIES AND RESPONSIBILITIES:
1. Assess patients waiting and develops a plan for all admissions and transfers where appropriate in collaboration with Centralized Bed Management.
2. Reviews and assigns patients utilizing the systematic room management process with Centralized Bed Management.
3. Collaborates with hospital departments in census management
4. Continually monitors bed availability to ensure bed status, utilizing alerts on system and on pager.
5. Monitors and facilitates unit discharge activity as projected.
6. Communicates with the Emergency Department, OR`s, PACU`s, unit management, staff nurses and physicians to facilitate the placement process.
7. Communicates with Patient Access and Scheduling ensure all patient placement needs are met.
8. Utilizes professional communication techniques when speaking to physicians, staff, patients and other departments in resolving patient placement issues.
9. Maintains and promotes patient confidentiality throughout the department.
10. Organizes work efforts during shift to assure smooth operations, and communicates to the next shift any ongoing information.
11. Completes shift census reconciliation and daily reports pertaining to census demand and bed capacity.
12. Identifies patient placement barriers and communicates to Centralized Bed Management.
13. Maintains morgue census and assists with autopsy consents when necessary.
14. Demonstrates the components of the ICARE values statements.
15. Follows all safety rules while on the job.
16. Performs all other job related duties as assigned.
EDUCATION REQUIREMENTS: High School Diploma or GED equivalent.
EXPERIENCE REQUIREMENTS: 6 months to 1 year of patient access experience, specifically in patient placement (or work experience in an equivalent position). Medical Terminology required.
