Teamwork and dedication as patient advocates are priorities for our Integrated Care Management Department! We offer tuition reimbursement for a variety of degrees, including a BSN. Our onboarding process includes training and mentoring with a focus on coordination of care.

As a Case Manager/ CM you will:

* Plan and coordinate care for patients by working collaboratively with the multidisciplinary team
* Provide ongoing support through comprehensive assessment, care planning, plan implementation and evaluation of patient needs
* Ensure patients are assigned the appropriate level of care, receive the appropriate services and are transitioned to the appropriate post-discharge level of care
* Perform accurate, timely and effective discharge planning for patients
* Work with third party payers to ensure coverage

Education and Experience

* Healthcare professional licensure required as a Registered Nurse
* Three years of experience in an acute care Medical/Surgical and/or ICU/CCU
* Ability to prioritize and organize tasks, time, and follow-up
* Certification in Case Management preferred, example CCM or ACM
* Knowledge of MCG Level of Care Criteria and Federal guidelines outlining coverage of inpatient and outpatient hospital services, including observation is preferred
* Knowledge of Medicare/Medicaid, insurance and regulatory guidelines is preferred
* Computer proficient in Microsoft
