The Health Information Tech/Scanning personnel play an integral part in the collection and preparation of medical records for patient care, coding and reimbursement activities, physician chart completion, and permanent record storage and retrieval. This position ensures that medical records are maintained in accordance with Medical Staff Rules and Regulations, State and Federal guidelines, and AOA & JC accrediting standards. All work is carried out in accordance with the Health Information Management department and CHS approved policies and procedures.

Successfully retrieves medical records from the nursing units and departments as necessary

Accurately prepares medical records for scanning by removing staples, paper clips, checking for color documents, clinical photos and any loose reports that need to be taped prior to scanning.

Verifies that each page of the chart has the patient name and account number.

Correctly attaches loose reports and dictation to the appropriate medical records by encounter date and number

Carefully denotes patient account numbers, date of service prior to scan of chart. Organizes charts by service type (Priority, Inpatient, Observation, Outpatient Surgery, Loose material and Color)

Scans and indexes by service type according to CHS productivity standards.

Carefully verifies what is scanned to the paper chart, when completed.

Effectively files completed charts to the designated file area by date of service and communicates any problems to supervisor.
