Whole Foods Market (WFM) has created a new and comprehensive model of medical care that aims to put our Team Members first and provide the highest quality care for both them and their families. Along with the Medical and Wellness Centers, Whole Foods now provides a support team to walk with Team Members throughout the journey from health to illness and back to health again. This personalized care team will follow Team Members through the primary medical care setting, to high risk medical care and specialists, even to hospitalizations if indicated. At our Medical and Wellness Centers each Whole Foods Team or Family Member is assigned to a Primary Care Team comprised of a primary care physician and a care coordinator. This new expanded team will always have the patientu2019s interest in mind , with the goal of improving care for our Team Members and their families, while supporting our culture of health and well-being. The Primary Care Team will establish a trusting relationship with their patients to accomplish health and wellness goals, encourage informed decisions on and improve the quality of the care prescribed, and improve overall quality of life.
Position Summary
We are looking for a full time Clinical Assistant to assist patients and the Care Team by providing clinical and care coordination/health resource navigation assistance. Please include a cover letter with resume.
Essential Duties and Responsibilities
Medical Support
* Ensure all patient reports, labs and information is filed in patientsu2019 medical records
* Maintain confidentiality of patient information and medical records
* Escort patients to exam rooms, interviews patients, measure vital signs, including weight, blood pressure, pulse, temperature, and document all information in patientu2019s chart u00A0
* Assist the physician or nurse practitioner with any on-site patient care needs within the scope of training and under the supervision of the primary care physician
* Keep exam rooms stocked with adequate medical supplies
* Perform blood drawu00A0
* performs injections, skin tests, and administering medication by inhalation
Care Coordination Support
* Professional Phone skills, maintains a pleasant and professional manner
* Schedule appointments, manage medical records and confidential documents
* Responsible for the daily communication and coordination of the basic medical needs of the patients, including researching, coordinating, and communicating medication refills and referrals
* Responds to phone and email messages in a timely manner; redirecting calls as needed
* Ensure smooth patient flow, timely care, and outstanding customer service for patients and their families
* Help promote timely access to appropriate care and increase utilization of preventative care.
* Maximize access to available resources, with the goal of enhancing patient health and well-being, increasing patient satisfaction, and reducing health care costs.
* Responsible for the daily communication, coordination, and implementation of all non-medical needs of their patients, as directed by the lead physician
* Continually assess and proactively seeking out potential issues in patients pursuant to the physician or Medical Directoru2019s instructions
* Responsible for presenting patient information at all huddles
* Work with patients and their families to ensure compliance to treatment plan
* Participate in outreach
* Understand and support WFM model of care and medical guidelines
* Maintains the confidentiality of patient information
Qualifications
* Medical Assistant Certification or LVN/LVN license obtained and in good standing
* 2+ years of administrative experience and/or clinical experience or equivalent in an out-patient primary care clinic setting
* Phlebotomy certificate
* Experience and training in general internal medicine, pediatrics, or family practice preferred
* Understands the benefits of coordinated care and building healing relationships u00A0
* Understands, or is willing to learn, WFM culture
* Able to deal with uncertainty and adapt to change in a positive manner
* Technological adeptness
* Excellent communication skills including verbal, listening and written
* Enthusiastic and compassionate, self-motivated with strong initiative
* Effective problem solver with excellent follow through
* Time management skills and ability to prioritize
* Hands on experience using electronic health records in an outpatient clinic setting
* Fluency in Spanish required
