Work Hours/Shift:

Part Time, Day Shift

You Will Be Responsible For:

* Processes all referrals to Rehabilitation unit for appropriateness, identifies discharge planning needs, ensures patients meet criteria for inpatient rehabilitation per CMS guidelines.
* Responds to all referrals promptly and communicates the decision to admit or deny to referral source
* Completes pre-admission screen with the required clinical information, verifying medical and rehabilitation needs, insurance coverage, and discharge plans.
* Upon receiving concurrence or denial from the Rehab physician, decision is communicated to patient, family and healthcare team.
* Evaluates patients needs for additional assistance with medical equipment, oxygen therapy, IV therapy, emergency equipment, pharmaceuticals need, specialty items etc. during the discharge planning process.
* Assures availability of equipment needed by providing ample time to the inpatient rehabilitation unit to order specialty item.
* Works in collaboration with UR, Case Management, Social Services, financial counselor, and insurance carriers to expedite patients discharge to the rehabilitation unit effectively and efficiently.
* Notifies the patient, family or healthcare surrogate of their financial obligations.
* Documents patient interviews, evaluations, recommendations, and summaries in the medical record.
* Demonstrates competency in all procedures within the scope of practice, as appropriate to the ages of the patients served. Maintains knowledge of growth and development with an understanding of the range of treatment needed by these patients.

Qualifications
What You Will Need:

KNOWLEDGE AND SKILLS REQUIRED:

* Understands and can communicate effectively with both external and internal stakeholders regarding the continuum of care.
* Must be able to multi-task and effectively manage multiple referral sources.
* Possesses excellent organizational and communication skills.
* Good computer skills, Word, Excel and Outlook.
* Clinical Chart Documentation / Review preferred
* Clinical / Medical Skills preferred

EDUCATION AND EXPERIENCE REQUIRED:

* Bachelors degree or higher in Nursing, Occupational Therapy, or Physical Therapy and three years of experience as a nurse, occupational therapist or physical therapist OR
* Associates degree in Nursing and five years of experience as a nurse
* Minimum of one-year experience in clinical rehabilitation setting preferred.

LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:

* Current valid State of Florida license as a Registered Nurse, Occupational Therapist, or Physical Therapist

Job Summary:

The Inpatient Rehabilitation Liaison is responsible for: Coordinating and assessing all activities related to the referral, evaluation, review and disposition of patients referred to the Rehabilitation Hospital. Assists in maintaining effective communication between the patient, caregiver, physician, contact person, admission office and the Rehabilitation Hospital during the referral and admission process, fostering positive relationships. Adheres to the Corporate Compliance Plan and to all rules and regulations of all applicable local, state and federal agencies, and accrediting bodies. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
