CareAllies Clinical Pharmacist

Job Purpose

The Pharmacist will play a critical role in executing CareAllies strategy in partnership with IPA, LLCs to strengthen their member physicians doctor-patient-community relationships and engagement by enhancing the quality and affordability of care. The clinical pharmacist will successfully service, monitor and grow strategic relationships with IPA, LLC leadership and their provider members by working side-by-side with IPA health care providers to help improve the quality, value and experience of care for their patients, to make health care better for everyone. This individual will focus on supporting providers and their office staff through performance management support, clinical program integration, and practice transformation.

The clinical Pharmacist will serve as the liaison to IPA physicians and to health plans. This role will work closely with Careallies Market Lead and Provider Engagement teams as well as several Health Plan departments, including: Market leadership, Health Services, Financial and the Data and Analytics teams.

Scope:

* Targets top opportunity physicians, delivers patient-focused data, and provides educational training regarding STARS and other population health clinical initiatives
* Attends IPA, LLC and physician meetings and provides education and guidance to physicians around various clinical topics
* Participates in quality improvement programs and initiatives driven by clinical needs
* Provide clinical support in partnership to Health Services Team on customer(s) outreaches through the various lines of business CareAllies supports
* Participates in the expansion of CareAllies offerings as they grow such as the Social Determinants of Health program
* Support the CareAllies Pharmacy Graduate Year (PGY-1) residency program

Job Components:

* Support physician practices by analyzing performance reports and data to guide decision-making, process, and program implementation
* Monitor and implement educational and process interventions based on practice-level data, trends and identified opportunities. Inclusive of, but not limited to:
* Partnering with IPAs and advising their primary care practices, physicians, nurses and other clinical staff to assist them on their conversion to value-based care
* Dissemination and support of quality performance data
* Testing and refining of workflows that support sustainable transformation to provider practices
* Guiding practices as they expand interventions to additional patient populations
* Responsible for utilizing available data to implement pharmacy initiatives, such as HEDIS/Star measures
* Utilize pharmacy benefit manager resources and maintain comprehensive knowledge base on all heath plan pharmacy benefits
* Participates in patient discussions as a member of the Careallies market multidisciplinary team
* Responsible for understanding and enhancing the CareAllies suite of clinical programs, identifying practice opportunities and integrating programs and resources into the practice workflows with the intent to optimize the patient experience, improve quality of care, and positively impact resource utilization
* Travel throughout designated territory to meet with physician practices in support of clinical practice transformation
* Serve as a drug information resource to Careallies internal/external customers
* Serve in the capacity of clinical preceptor to provide oversight of students and/or residents to execute specific outreach programs including HEDIS/Star measures, and other initiatives
* Duties as assigned

Qualifications:

* Pharm. D. degree or equivalent education in pharmacy
* Completion of a post graduate clinical training, such as a PGY1 pharmacy residency program, is required
* 1-2 years of managed care experience strongly preferred
* Demonstrated ability to work independently, solve problems, and make sound decisions in a timely fashion
* Ability to deal with ambiguity and continue to operate effectively, even in situations in which the way forward is not clear
* Effectively builds informal relationship networks inside and outside the organization
* Very strong communication skills to convey clear understanding of unique needs to different audiences
* Proficiency in Microsoft office platforms including word, excel, and power point
* Current resident pharmacist license in good standing
* 25% travel
