Work Hours/Shift:
Full Time
You Will Be Responsible For:
* Interviews patient, family members and significant others involved with the patient to determine potential bio psychosocial and discharge needs.
* Develops and implements discharge plans and supportive care for patient based on assessment and collaboration with the treatment team (inclusive of RNCM, RN, MD and therapists). Monitors and revises the discharge plan as needed to affect a timely discharge of patient that adequately meets patient's needs.
* Provides patient and/or significant others with information and community services and resources available and completes referrals as needed. Completes mandatory referrals to abuse hotline as required by statute.
* Provides crisis intervention counseling and supportive care for patient and significant others during hospitalization.
* Participates in interdisciplinary rounds with the hospitalists
* Participates in plan of stay rounds and provides length of stay and discharge planning information. Coordinates with the Charge RN/ RN/ and/or hospitalist for daily interventions to improve patient flow.
* Communicates with patient and significant others in an effective manner and facilities communication between the patient and treatment team.
* Completion of/attendance of ethic consults meetings
* Actively involved in advocacy for the patient population and profession by functioning as a social work representative on committees, providing coaching for peers and demonstrating professional behavior (as defined by Code of Ethics)
* Completion of 30-Day Readmission Tool on all Inpatient readmissions within the last 30 days.
* Completion of High Risk Assessment based on LACE Tool score of greater than 10 that is completed by the RNCM on all Inpatient's
* Provides quality customer service to external and internal customers both on telephone and in person to ensure courteous and professional experience
Qualifications
What You Will Need:
* LCSW/LMHC and/or LMFT.
* One (1) year of case management experience required for an LCSW and two (2) years in an acute care setting required for all licensures
* Current FL LCSW, LMHC, and/or LMFT license required
Job Summary:
Utilizes developed medical social work skills to assess patient and family functioning and emotional well-being in order to develop appropriate and timely discharge plans, community referrals, and provide psychosocial support. In collaboration with the physician, nursing, therapy staff and RN Case Managers, provides assessment, planning and interventions to optimize quality of care, utilization , costs and patient satisfaction. Demonstrates uses of advanced skills to provide detailed assessments, interventions and advocacy for patients and the profession. Evaluates patients who exhibit psychiatric issues as requested and functions as LCSW Proxy for health decisions as requested by designated acute care hospitals.
