In this role a Maternity Case Manager will educate expectant families, coach for lifestyle change when appropriate and screen for high risk situations. Telephonic interactions with members are structured around pregnancy and post-partum education and screening. Health coaching for behavior change and self-advocacy are also key tools in this role.

This is a work from home position working with the Healthy First Steps Program. Employee must reside in Green Bay, WI area.

Primary Responsibilities:

* Effectively screens for high risk maternity situations
* Coach and motivate member to take action to reduce risk of pregnancy and post-partum complications
* Establish trust and build long term relationships with expectant mothers
* Support the care needs of the mother-baby dyad during and after a NICU admission
* Support safe discharge home and transition of care post-discharge and reduce NICU readmissions for our smallest members
* Provide information about common comorbidities, other health conditions that may affect NICU baby's discharge planning and transitions of care
* Provide individualized member health education and link member / family to appropriate community resources including but not limited to, information / referral to sources of financial assistance, transportation, support groups, and other community services
* Care coordination and collaboration with community partners
* Serve as member advocate and liaison with physicians, families, community agencies and others as needed to ensure continuity of care
* Modify the plan of care, as appropriate, based upon reassessment, the member need for further care or services and the achievement of identified goals
* Utilize established standards of care to identify and address members' gaps in care
* Participate in rounds with the Medical Director and discussing cases as needed
* Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction
* Visit members in their homes and/or other settings, including community centers, hospitals or provider's offices

Required Qualifications:

* Current, unrestricted Wisconsin RN license
* 4+ years of clinical experience
* Clinical experience in obstetrics, pediatrics and NICU
* Computer / typing proficiency to enter/retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applications
* Access to high speed internet from home
* Dedicated work space from home
* Access to reliable transportation that will enable you to travel to member's home within a designated area
* You will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role
* Employees are required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener prior to entering the work site each day, in order to keep our work sites safe. Employees must comply with any state and local masking orders. In addition, when in a UnitedHealth Group building, employees are expected to wear a mask in areas where physical distancing cannot be attained

Preferred Qualifications:

* Bachelor's degree or greater
* Case Management experience
* Certification in Case Management (CCM)
* Experience working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needs
* Bilingual skills
