In the Neonatal Resource Services program, this position provides telephonic support to families receiving services through the Neonatal Intensive Care Unit (NICU) with Monday through Friday work schedule. This will be a telecommuting role, based out of the employee's home.

You'll enjoy the flexibility to telecommute* from anywhere within the U.S. as you take on some tough challenges.

Primary Responsibilities:

* Educating and supporting the family unit through telephonic assessment and expert clinical interventions
* Evaluating delays in care, facilitating discharge planning, and arranging for alternative care services to facilitate timely and appropriate discharge
* Effectively using established operating systems to document activities and ensure consistent program clinical practices
* Collaborating with hospital staff and other clinical programs to enhance the quality and efficiency in healthcare delivery
* Making outbound calls to assess members' current health status
* Identifying gaps or barriers in treatment plans
* Providing patient education to assist with self management
* Interacting with Medical Directors on challenging cases
* Coordinating care for members
* Making referrals to outside sources
* Coordinating services as needed (home health, DME, etc.)
* Educating members on disease processes
* Encouraging members to make healthy lifestyle changes
* Documenting and tracking findings

Required Qualifications:

* Active and unrestricted RN license in your state of residence
* 2+ years of high risk level II or level III NICU care experience
* Proficiency utilizing Microsoft Word, with the ability to navigate a Windows environment
* Dedicated, distraction-free work space within the employee's home with access to high speed internet
* If you need to enter a work site for any reason, you will be required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener. When in a UnitedHealth Group building, employees are required to wear a mask in common areas. In addition, employee must comply with any state and local masking orders

Preferred Qualifications:

* Bachelor's Degree
* A background in managed care
* Case Management experience
* Certified Case Manager (CCM)
* Experience / exposure with discharge planning
* Experience in utilization review, concurrent review and / or risk management
* Strong organizational skills and multitasking abilities will be keys to success
* Superior communication skills

Colorado Residents Only: The salary/hourly range for Colorado residents is $53,300 to $95,100 (add additional reference for sales and physicians).
