Senior Wellness Coordinators provide telephonic care coordination for enrollees.

You'll enjoy the flexibility to telecommute* from within the state of Washington as you take on some tough challenges.

Primary Functions

* Make outbound calls to members who are identified with Behavioral Health needs.
* Complete telephonic assessments to identify care needs.
* Contact providers and document aftercare appointments in system.
* Contact members post-hospitalization to ensure understanding of aftercare plan and provide assistance with any additional behavioral health needs.
* Assist member with appropriate community support services.
* Partner with licensed Care Coordination Advocates on member support and advocacy.
* Other duties as assigned.

* Excellent customer service and advocacy skills
* Excellent interpersonal and problem-solving skills with the ability to use community and network resources to meet members' needs.
* Strong oral and written communication skills
* Strong spelling and ability to type 30-40+ wpm accurately
* Excellent time management with the ability to be flexible in a dynamic environment
* Effective at organizing, prioritizing, and following through on tasks in a timely fashion
* Strong team player with a willingness to learn
* Able to manage confidential issues with members and providers
* Initiative in achieving individual, team, and organizational goals and objectives

Required Qualifications:

* High School Diploma / GED
* Live in state of Washington
* 2+ years of Managed care experience
* 2+ years of customer service experience
* 2+ years of Medical / Behavioral setting experience (i.e. hospital, managed care organization, or joint medical / behavioral outpatient practice)
* 2+ years of community-based resources and services.
* 2+ years of Microsoft Office experience (including Word, Excel, PowerPoint and Outlook)
* If you need to enter a work site for any reason, you will be required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener. When in a UnitedHealth Group building, employees are required to wear a mask in common areas, In addition, employees must comply with any state and local masking orders.

Preferred Qualifications:

* Bachelor's Degree (or higher) in social work, psychology or related field
* 2+ years of Dual diagnosis experience with mental health and substance abuse
* 2+ years of Experience working in an environment that required coordination of benefits and utilization of multiple groups and resources for patients
* 2+ years of Experience working with low - income populations
* 2+ years of Experience working with the aged, blind or disabled
* 2+ years of Experience working with the Medicaid population
