Providence Health is currently looking for full-time Social Worker/Discharge Planner to join our Case Management team. The ideal candidate will have previous discharge planning experience within an acute care or home health setting.

Essential Functions

* Assesses within 24 hours, patients referred by physician, nurse, case manager (s) or patient's identified as high risk as defined by department policy. Assesses and documents in medical records psychological, emotional, psychosocial, and financial data to determine the patient's post-hospital needs.
* Reassesses discharge plan and updates documentation with patient progress in medical record until final disposition
* Facilitate and transfer as required by case manager(s) to include but not to be limited to the following healthcare providers: Acute Care, Long Term Acute Care, Acute/Sub-Acute Rehabilitation, Mental Health, and External Care Facility. (ECF) Execute discharge planning for patients requiring complex discharge healthcare needs, and/or significant psycho/social needs.
* Collaborates with physician and members of the interdisciplinary team to obtain input regarding discharge plan.
* Formulates a plan the encourages continuity and coordination of health care upon discharge from the hospital.
* Documents final disposition and destination at time of discharge and communicates information to case manager, MD and designated family members.
* Counsels with patient/family to ensure acceptance, appropriateness and continuity for the plan of care
* Is adept at crisis intervention involving patients and families experiencing psychosocial problems
* Identifies and remains current on eligibility requirements, regulatory statures, post-acute providers, and availability of community resource in regards to attainable patient outcomes.
* Maintains good rapport and working relationship with internal and external resources.
* Performs follow-up with discharged patients on an as need basis to determine effectiveness of the discharge plan and to address identified problems related to service delivery.

Job Requirements

MinimumEducation

BSWrequired, MSW or MRC preferred

Mustbe licensed to practice in the State of South Carolina

BLS

Minimum Work Experience

Minimumof three years experience in social work/discharge planning/utilizationreview/vocational rehabilitation practice preferred.
