Ennis Regional Medical Center
Monitors and manages patient care to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance.
Consults with nursing staff and multidisciplinary team regularly to evaluate patient's status and appropriateness of medical care, including admission, length of stay, transfer and discharge.
Monitors patient and family satisfaction. Responds to questions and complaints from patients, family members, and payors regarding care.
Participates in discharge planning including coordinating patient transfers to other facilities and coordinating community resources. Provides discharge education and resource referrals to patients.
Performs chart review to identify actual or potential issues with service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.
Regulatory: Maintain compliance with national standards and regulating bodies such as Joint Commission, CDC, OSHA, FDA and CMS. Participate in maintaining and developing policies and procedures.
House Supervision and Staffing: Provides support to staff, visitors, patients and families, and physicians to meet operational needs. Insures adequate staffs are in place to provide safe and effective care.
Satisfaction and Advocacy: Insures patients complaints or concerns are addressed in a timely manner. Focuses on patient, family, and physician satisfaction using a planned and systematic approach.
Quality and Performance Improvement: Mange trends and outcomes as part of the Care Management team; assist in Risk and Case Management and other operational improvement efforts. Improves overall quality and completeness of clinical documentation by performing admission / continued stay reviews. Conducts timely follow-up reviews of clinical documentation to ensure that issues discussed and queries left in the medical record have been answered by the provider. Tracks and trends specific opportunities for improvement to include complication and mortality analysis and interventions as part of the Patient Safety Culture Initiative. Analyzes resource reports and guides facility in improvements based on results.
Job Requirements
High school diploma or equivalent X Required
BSN Preferred
Five years of experience in a clinical role and two years of experience in a supervisory capacity, case management or performance improvement experience preferred.
Prior experience in case management, clinical documentation improvement, house supervision, infection control, quality or risk management preferred.
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
BLS required
ACLS within 3 months of hire
PALS within 3 months of hire
State of Texas current RN license
